GHI's PPO Plans for Sole Proprietors
GHI's Preferred Provider Organization (PPO) plans, available to small businesses throughout New York State, offer a choice of health care within GHI's tri-state network of 142,000-provider locations at a deeply discounted rate, or outside our network at a higher rate. You don't have to select a primary care physician, and you don't have to get a referral from your doctor to see a specialist in the network.

The GHI PPO program makes it easy for members to use their benefits, while meeting their need for quality, affordable health care, with a wide spectrum of medical, hospital and prescription drug benefits. As a special program feature, group members have access to the GHI Employee Assistance Program (EAP).
Competitive cost-saving options make it easy for groups to get the right coverage at the right price. Two- and four-tier rate options are available and guaranteed for a full year.
Prescription drug coverage is a feature of the GHI PPO plans. You can choose from one of many plans, with multiple
copayment options.
There are no claim forms to file when members see participating providers. And with 24/7 customer service, members enjoy "anytime anywhere" access to provider, eligibility, claims status and benefits information at GHI's Web site, or by calling our automated phone system, the GHI
AnswerLine.
