At GHI, we want to help you manage your medical condition and team up with your doctor to enhance your quality of life. We have programs, focusing on specific diagnoses, to do just that. In fact, we offer a variety of diagnosis-specific programs based on your plan type. Under our PPO plan, GHI programs for specific diagnoses can help you manage your condition and team up with your doctor to enhance your quality of life.
Manage Your Condition - for PPO MembersArthritis, Depression, Diabetes, Osteoporosis
Disease Management Programs are voluntary and available to all eligible members who qualify, based upon diagnosis. Members may enroll themselves or may be referred by a GHI HMO provider. Members can also be identified as a candidate through welcome calls, health risk assessments, claims reviews and/or pharmacy data.

GHI Medicare Choice PPO offers disease management programs at no additional cost. Plus, our Care Enhance programs can complement the care you receive. Among the services we offer you are scheduled calls, round-the-clock service and ongoing education.
Our Care Enhance programs can help you:
- Improve compliance with your doctor's instructions and treatment plan
- Improve your understanding of your condition
- Improve and/or stabilize your symptoms
You can be referred into these programs through Health Risk Assessment or by your doctor. Or, simply call toll-free 800-622-9531 for information, or to refer a GHI Medicare Choice PPO member into our Diabetes, COPD or Heart Failure program.
