L

A license or permit (or equivalent) to practice medicine or a health profession that is 1) issued by any state or jurisdiction in the United States; and 2) required for the performance of job functions.

The maximum accumulated payments your health plan will make for covered services rendered to a covered person per lifetime, or the maximum number of days/visits available to a covered person per lifetime for a particular service or services. As of their policy renewal date on or after October 1, 2010, members will have no annual or lifetime dollar limits for essential benefits.
