In-network plan delivered at physician group practices, which offer multiple services in one convenient location, or by solo practitioners. Primary care physician (PCP) referrals required or recommended depending on plan selection. Customers can also select a non-referral based HMO or POS product.

For New York-based employer groups for their employees who reside primarily within the following 10 counties: Bronx, Kings (Brooklyn), Nassau, New York (Manhattan), Orange, Queens, Rockland, Staten Island, Suffolk and Westchester counties.

No gatekeeper or referrals required

Coverage with a copay or deductible and coinsurance. Deductible and coinsurance are applied to facility services

Optional rider available for purchase that provides up to $200 annual reimbursement for health club membership, as well as an enhanced vision benefit and alternative medicine services, including acupuncture, massage therapy and nutritional counseling

Optional benefit riders are available for purchase which may contain a deductible and coinsurance

Age 29 Law Rider extends the age of dependency to age 30 (end of month), allowing the group's eligible members' dependents to remain covered under the member's plan.
