New streamlined process for dependent student verification
In keeping with our commitment to make it as easy as possible for your group members to get the most out of their coverage, GHI has streamlined procedures for verifying dependent student eligibility.
In the past, subscribers were required to obtain a school registrar’s signature and seal as proof of their dependent student’s eligibility. Instead, GHI is now placing with the subscriber the full responsibility for verifying that his or her dependents are eligible for GHI coverage as full-time students.
To verify student status, subscribers simply download and complete the new GHI Student Verification Parent Affidavit Form, available through GHI.com. This form requires that the subscriber attest that his or her dependent meets all GHI’s eligibility criteria, which stipulate that the dependent must be:
- At least 19 years of age.
- Under the age limit for the subscriber's GHI plan.
- Unmarried.
- Dependent on the GHI subscriber for at least half of his or her support.
- A full-time student in an accredited secondary or preparatory school or college.
The parent also needs to indicate on the form the student’s expected date of graduation, sign the form, and return it to GHI. The GHI Student Verification Parent Affidavit Form is available for immediate use. To view or download a copy, click here. |
