You’ve looked at your options, and now you’re ready to enroll. We want to make the process as easy as possible for you. This section outlines your three EmblemHealth Medicare Advantage Plan enrollment options.

Members may enroll in the plan only during specific times of the year.

Medicare beneficiaries may also enroll in an EmblemHealth Medicare Plan through the CMS Medicare Online Enrollment Center located at: http://www.medicare.gov.

Medicare beneficiaries may also enroll in an EmblemHealth Prescription Plan through the CMS Medicare Online Enrollment Center located at: Part D CMS enrollment

Make an in-home appointment

Call us toll-free at 1-800-447-9169, 7 days a week, 8 am to 8 pm, (TTY/TDD users call 1-877-444-2786) to:

• Schedule an in-home appointment with an EmblemHealth Sales Representative who will answer your questions and help you complete the enrollment application.
• Request an enrollment kit.

Medicare beneficiaries may enroll in an EmblemHealth Medicare Advantage Plan for 2013 over the phone beginning October 15th by calling us toll-free at 1-800-447-9169, 7 days a week, 8 am to 8 pm (TTY/TDD users call 1-877-444-2786).

Enroll by Mail

Medicare beneficiaries may enroll in a plan by downloading an enrollment form and mailing it the address below. You can download either the Medicare Advantage Application or the Prescription Drug Plan (PDP) application.

Direct Debit Program

EmblemHealth offers a Direct Debit program, making it easier to pay your monthly premium payment. Each month, EmblemHealth deducts your premium directly from your bank account. There are no monthly premium invoices and no premium checks to write. The Direct Debit program saves you time, paperwork and the worry that you will forget to pay your premium. If you would like to sign up for the Direct Debit program, please complete the Direct Debit Program Application Form (Y0026_121903 File & Use 07/28/2010)and mail it in with your enrollment application.
